Services What we do
Four services, one team behind all of them.
Everything we provide is outpatient or community-based — you live at home. Most people use more than one of these at a time, and the combination changes as your life does. Here's each one in plain language.
01 Outpatient
Outpatient treatment.
Counseling that fits around your life instead of replacing it. You come to us on a schedule that works with your job and your family, and the intensity adjusts to what you actually need — more support during hard stretches, less as things steady out.
Sessions cover what's really going on: stress, family strain, mental health symptoms, substance use, and the places where they overlap. Alongside counseling, we teach practical education on wellness and recovery — the skills that keep progress going between sessions.
What this includes
- Individual counseling, one-on-one
- Group sessions with people working on similar things
- Family counseling, with your permission
- Education on wellness and recovery
- Varying levels of intensity based on your needs
A good fit if
- You want regular support while living at home
- You're managing mental health, substance use, or both
- Your family wants to be part of the picture
02 Coordination
Case management.
One qualified professional who knows your whole situation and helps you move through it. Getting help usually involves a maze of appointments, benefits, paperwork, and systems that weren't designed to be easy — and that maze is exactly where a lot of people give up.
Your case manager handles the coordination: assessment, planning, and connecting the pieces so nothing falls through. The goal isn't to do everything for you — it's to build your self-sufficiency and independence over time, with occasional counseling support along the way.
What this includes
- Assessment of your situation and needs
- A plan built around your goals
- Coordination of appointments and services
- Help navigating benefits and paperwork
- Occasional counseling as part of the support
A good fit if
- The logistics of getting help have stopped you before
- You're juggling multiple providers or systems
- You want one consistent person in your corner
03 Real world
Community integration.
Skill-building that happens out in the actual world, not just in an office. Living well in your community takes a set of practical skills — holding down work or school, keeping health appointments, filling free time with things that are good for you — and those skills are learned by doing.
Activities are tailored to your needs and goals. You participate actively in real settings, covering recreation, vocational pursuits, education, and health, so that community living isn't just possible — it's workable, day after day.
What this includes
- Skills for successful community living
- Support around work and vocational goals
- Education-related support
- Help staying on top of health and appointments
- Recreation and community activities
A good fit if
- You're rebuilding routines after a hard stretch
- Work, school, or daily structure feels out of reach
- You learn best by doing, with support nearby
04 Long-term
Supported living.
Long-term support for adults living in their own homes. The home is yours — owned or rented by you — and we come to it. Support is shaped around your desires and needs, not a facility's schedule.
Because it's long-term, the support adapts over time. What you need this year may not be what you need in three years, and the services adjust as your life changes. Occasionally we can also assist with housing itself when that's the barrier.
What this includes
- Ongoing support in your own home
- Services shaped by your desires and needs
- Adjustments over time as needs change
- Occasional assistance with housing when needed
A good fit if
- You want to stay in your own home with support
- Your needs are ongoing rather than short-term
- Independence is the goal, at your own pace
Next Not sure which?
You don't have to pick from a menu.
Most people use more than one of these at a time, and figuring out the right combination is our job, not yours. Call, tell us what's going on, and we'll tell you what we'd recommend — including being honest if another provider is the better fit.
Getting started is three steps: a phone call, an assessment, and a plan built around your goals. Nothing is committed to until you say so, and the first conversation costs nothing.
